How to generate Assembly drawing with designators inside the boxes like this in Altium? Is it by placing designators (silkscreens) inside manually? I guess not.

What layer is the outline coming from?
There's a checkbox output option to copy certain layer(s) onto all output plots.
"Mechanical Layer(s) to add to all Plots". Maybe the layer with the outline is checked.
You can delete the text selecting "Format" equal none. Then you can use the "Prefix" and "Suffix" menus to add a specific text.
The "Sample" box will be updated accordingly.

Altium 16.1 includes "Draftsman" documents, and there is a view that does this automatically. Before this version, I used Photon's answer to create a similar drawing, and while it did take a while, the results were very useful.
