I have a plastic crusher with a three phase motor and I will like to operate it using single phase 220v.
Can anyone please help me out with instructions on how to connect it?
Also, I want to know if I will be using two capacitors?
(start capacitor and a run capacitor)
Can I also know the exact capacitor(s) value which I will require?
I have attached a picture of the label of the motor, a picture of the motor contactor and a picture of the motor wiring.

and 2.: The terminals 4, 5 and 6 are not to be connected to your VFD. This is indicated by the drawing, as there is no wire "leaving" the interconnected terminals. i.e. those are bridging terminals to build the correct wiring of all windings of the motor.
probably yes. Normally PE connectors should be designated by green/yellow, but I've seen it often as only green.
Depends from your wiring scheme on your premises. If you have combined neutral/protective-earth connect that to your VFD and your motors PE. If you have separate neutral / protective earth you have to connect the PE to your motor's PE terminal.
The PES is for EMC-purposes. It starts at your motor's housing and is connected back to the shielding cabling between the motor and the VFD. It will reduce electromagnetic interference. For this purpose you have to use shielded cables. Is it necessary? Probably you won't get any problems with not shielding it. But others may. So in my eyes not applying the appropriate measures for EMC is like letting my dog shit onto the green of my neighbours. So: just do it and use shielded wires. According to the datasheet your motor cable should be low capacitance (<75 pF/m wire-wire, <150 pF/m wire-shield).
It is a single-phase motor. Any single-phase motor can be run from a 3-phase supply of the proper voltage. Just connect it to any two phases. Are you sure that you connected capacitors of the proper microfarad value to the proper terminals? Are the capacitors rated for motor-start and motor run duty?
The diagram below shows what I think are the internal winding and starting switch connections. C2 must be the starting capacitor. It is not shown to be connected to the power connections, therefore it must be connected to an internal component that disconnects it after the motor reaches full speed after the power is switched on. Since the stating capacitor has a high capacitance, it allows a high current in the start winding. It is designed to carry the high current for only a about second or two. If it remains connected, it and the start winding will overheat and fail. C2, the start capacitor, should be the 250 uF capacitor and C1, the run capacitor, should be the 50 uF capacitor.
The reversing links change the polarity of the start winding with respect to the run winding. The connection diagram should show the links in only one position, not both at the same time. The motor must have the direction selection changed only when it is unpowered and not turning.

If your budget allows it you should get a VFD (Variable Frequency Drive). It can create three phase signals with different phase shifts and frequencies, giving you the ability to cintrol speed, direction, power etc. of your motor. These can be found on eBay for moderate prices, the more powerful they are the more expensive. There are probably other options, but considering the 3 phases are kinda crucial for the motor to work, I don´t think you can just connect it to a single phase without some kind of motor controller.
