I thought it was called a "jog dial" but I can't any parts by that name.
There are five pins on the back and one on each side.
What is it called and how can I find a replacement?

It's a female crimp terminal (sometimes called an open barrel or u-barrel crimp connector). More specifically, since it has the plastic housing, it's called a PCB female crimp terminal.

If the case was that you happened across this image out of context, I recommend images.google.com. Google lets you search by image, basically you can bypass the 1000 words by just uploading the image.
As mentioned, these spacers are not sold separately (not as a standard part, anyway). However, you can buy headers just like the one you showed in your first picture with the spacers already added. I know Sullins sells some connectors like this with the option of adding these spacers within the order. It's part of the part number configuration. See the catalog page for the PPPC series (http://sullinscorp.com/catalogs/101_PAGE114-115_.100_FEMALE_HDR.pdf). In the fourth table on the first page you'll see the different options for spacers and tail length. I think this is probably what you're looking for. The top of page 2 shows how to configure the part number.

The generic name for this seems to be: 5-way navigation switch.
Using google I looked at a lot of pictures of this type of switch but did not spot one that use 5 SMD terminals all in one row like your pictures show.
