Magento – Add custom form value with product

I want to create a form which have some conditional logic.
I tried hard but did not found any free plugin/module to do this.

So finally have a solution that I will create custom form(That will create by myself not magento's custom option form) to do that task. But didn't know How can I add this form values submitted by user when he add that product to cart. And also admin can see the values on the admin panel as well.

I know magento have 'custom option form' associated with product but I m not able to add conditional logic on that form. So How can I submit my form as 'custom option form'.

I don't understand what your problem is.

In magento a configurable product represents a group of simple products. When you add a configurable to your cart, in reality you are adding a simple product to your cart with another label.

When you omit the choice of the attribute, the system don't know what to add to the cart, so do I.

What is the intended behavior when you don't choose any option?

Got it working now, what I've done is using jQuery to hook on myForm.submit() action as follows:

var myForm = new VarienForm('my-form-id', false);
var formAlreadySubmitted = false;
$j('#my-form-id').submit(function(e){
   if(myForm.validator.validate()){
      if(formAlreadySubmitted){
         e.preventDefault();
            return false;
         }
         var submitChildren = $j(this).find('button[type=submit]');
         submitChildren.attr('disabled', 'disabled');
         submitChildren.addClass('disabled');
         formAlreadySubmitted = true;
    }
});

Notice that I'm only disabling the submit button if the form passes Magento validation, so to give them the chance to correct the form if it's invalid.
