I have reduced setup to just 2 computers connected via switch with fixed IP addresses.
PC-A is able to connect to or ping PC-B
PC-B is not able to connect to or ping PC-A
Windows firewall is disabled on both PCs.
How do I find out what program is blocking incoming connections to PC-A? Because there is no HW that will block connections, there must be some software blocking that, but I have no clue how to find it.

Add a LAN Firewall rule to block the IP of the guy by going to Firewall -> Rules -> LAN:

And be sure your rule is before the default "allow everyone" rule; since rules are processed top down, in order, until it finds one that matches.
If the ICMP packet was sent from 192.168.4.37 to 192.168.5.x and the reply is received from 192.168.4.25 I wonder if this could be considered a spoof. In the m0n0wall advanced settings I notice these 2 options:
I don't know the inner workings of m0n0wall. But I wonder if there are system firewall rules added that are not actually shown in the rules web interface that may be blocking the echoreply.
Can you goto /exec.php and post the output of command:
ipfw list
This would probably be of help in debugging.

Are these clean Windows install, or are they company image? Other common software that blocks traffic include AV, HIP etc. Disable them - or go into safe mode and try, then re-enable one by one to find the culprit.
