I've run in release mode my app on a iPhone with Xcode 4.
Everything worked great until when I switch to the simulator and I switched back to the iPhone. After switching back, the app is launched on the iPhone, but the home screen is displayed immediately after and in Xcode I get:
failed to get the task for process
I've tried to remove/add Entitlements file, delete app and restart Xcode but nothing to do.
I've had this error many times. While TechZen's answer is absolutely right in this case, another common cause is when you change the name of a IBOutlet property in your .h/.m which you've already connected up to File's Owner in the nib.
From your nib:
Under 'Referencing Outlets' make sure that your object isn't still connected to the old property name... if it is, click the small 'x' to delete the reference and build again.
Another common cause if you are using Storyboard, your UIButton might have more then one assignings (Solution is almost the same as for nib):
You will see that there is more than one assign/ref to this button. Remove one of the "Main..." greyed windows with the small "x":
For those reading this in regards to Xcode 4.2, and attempting to run on an earlier device (e.g. iPhone 3G, 2G, iPod 1st gen, etc) I have another solution. New projects created in Xcode 4.2 by default specify 'armv7' in the 'Required Device Capabilities'. You'll need to remove this if wanting to support devices that run armv6 (e.g. the iPhone 3G).
Delete armv7 from the 'Required device capabilities' in yourProjectName-Info.plist
You may also need to change the build settings to compile with armv6 instead of armv7.
This is the default:
Double click on 'Standard (armv7)' to add another, then click the '+' in the popup, and type in 'armv6':

I am betting that your release mode includes compiling with your distribution certificate, which disallows this behavior (you wouldn't want some random fool hooking into your app after downloading it from the app store). Compile with the development certificate instead. You can change this in the building settings under code signing.
