This is probably very similar to other questions asked, but I would like a little further clarification. If I upload a photo and my friend comments on it, does this photo then appear in their friends news feed along with their comment and any other comments/likes by me or my other friends? My privacy settings are to my friends only.

Are you referring to your News Feed (Top News), News Feed (Most Recent), or the Recent Activity on your profile?
I think that News Feed (Top News) only displays what Facebook considers to be most interesting to you, so I wouldn't expect your own likes/comments there. The News Feed setting that you mentioned applies to the Most Recent setting.
News Feed (Most Recent) does not display individual stories for comments and likes on posts; you would only see them under the corresponding post, if it is expanded. Are you saying that you expanded the likes/comments and you can see comments from other people but your comment is not there? Sometimes it takes some time for it to show up although it shouldn't take days (unless it was deleted).
If you are referring to the Recent Activity on your profile, there is an additional setting for that that you didn't mention. On your profile's Wall (not your News Feed), scroll to the bottom and click Edit Options. If you have hidden any kinds of stories from your Recent Activity these will be listed and you can unhide any of them by clicking the X and then Save.
As for the Photos tab, that used to be a known issue although the link to Facebook Help in that answer does not seem to work now. I don't know if that means that it should be fixed.
The visibility of your comments and likes is dictated by the person/page you are liking or commenting on. If you comment on a friend’s post that is visible to Friends, then all their friends will be able to read that comment. Furthermore, your comment may appear (based on Facebook’s relevance algorithm) in the news feeds of those friends you have in common.
This extends to any kind of visibility assigned to the post that you like/comment on. So if they have set visibility to Friends of Friends then your like/comment will be visible to their friends’ friends; once again if that group encompasses your own friends, then your action may appear in their news feeds.

No. Only your friends will still see the picture.
Any comments on an item inherit the same privacy as the original item, and never change the privacy of the item.
If your privacy setting for pictures limits the visibility to only your friends, the friends of your friends won't see it, or any comments on it.
However, if they are also your friends, they might get in their news feed either the picture, or a mention of your friend's comment on the picture (depending on what the omnicient holy Facebook Algorithm determines they want to see).
