Using Google Docs Forms, I'd like to create a checklist with a "quantity" and "comments" textfield adjacent to each checklist item. Is it possible to do this without writing a custom script?

The answer validation in Google Forms could be done with Google Apps Script after the students submit their responses, i.e. by using onFormSubmit event.
Once the answer was submitted, you could use the Class File to get properties like:
- Viewers
- Editors
- Owner
It's worth to say that in the 2014 summer Google launched Google Classroom to help teachers to manage the assignments of their classes among other tasks.

Currently, we can't time a quiz per se using Google Apps Script. However, you can do a few things like limit how many responses a Form can take or put a hard stop time on the Form.
However, there is a free Add-on:
I would envisage some inherent problems with any timer due to differences in download speeds and internet unavailability.

At this time it's not possible to change the the default form layout directly from the Google Forms editor.
Alternatives
Use HTML Service of Google Apps Script to create a web app and submit the answers to a Google spreadsheet. Further details are out of the scope of this site, maybe you could get help on this at http://stackoverflow.com. `
