I've a Google Form which includes a multiple choice list with radio buttons and would like each item to be a hyperlink to an item description.
Is it possible? How?
If not, is there a workaround?

In your spreadsheet copy the column of cells with your choices. (Don't copy the whole column.)
In the form's multiple choice question select "Option 1".
Paste in your cells. (CTRL-V or CMD-V on Mac)
To get Radio Buttons on a Google Docs Form, you need set the Question Type to Multiple Choice

Google Forms will not accept aliased links. This is set by default by Google as a security measure.
You can either add the links/hyperlinks in the section title description like:

Add the links directly as choices
You can also use a URL shortener
