I'd like to send Evernote reminders from a Card. Is there a way to customize the email template used in notifications (ie: set the subject to something different from 'Trello Notification'), so that Evernote can assign the Card in the appropriate Notebook?

This is a common misunderstanding of the way Trello notifications work, not a bug. We only notify you on stuff you don't see. So if you keep a browser window open with Trello on it, you won't get notifications (for now).
Subscriptions to boards and cards, which is forthcoming, should address the need for email notification on new actions. For now, notifications only notify on stuff that happens while you're away.
An easy way to do this might be with this Zap from Zapier.
It will post a message to Flowdock for any new Trello card activity (which is essentially card changes).
Disclaimer: I am a founder at Zapier, but this is exactly what I'd do even if I wasn't.

Sorry, no, Trello doesn't offer customizable emails. We are working on an API, however, which would allow you to integrate with Evernote.
